Most cramping in pregnancy is the uterus doing what it has to do — growing, and dragging the ligaments that hold it with it. That is genuinely reassuring and it is not where this page starts, because two of the conditions that present as cramping are time-critical and reassurance read first tends to be the part that sticks.
Call your maternity unit or emergency services now if: the pain is severe and one-sided, especially with pain at the tip of your shoulder, faintness, dizziness or feeling generally very unwell — that can be an ectopic pregnancy and it does not wait. Call the same day if: cramping comes with any vaginal bleeding; cramps become regular and painful before 37 weeks; you have a gush or trickle of fluid; or you have a fever alongside the pain.
The short answer: mild, low, central cramping that settles with rest is normal in the first trimester — it is the uterus stretching. In the third trimester, irregular tightenings that come and go are usually Braxton-Hicks. The features that make cramping abnormal are not severity but bleeding, one-sidedness, and regularity before 37 weeks.
Why it happens
The uterus more than doubles in size in the first trimester alone, and it is anchored by ligaments that have to lengthen with it. That stretch is felt as low crampy discomfort, sometimes as a pulling sensation, and it is more noticeable after activity because movement puts the ligaments under load. A full bladder or a constipated bowel sits directly against the uterus and adds to it, which is why the two often travel together.
Later in pregnancy the uterus starts practising. Braxton-Hicks contractions tighten the whole bump for 30 seconds or so and release. They are irregular by definition, and the moment they stop being irregular they stop being Braxton-Hicks.

Time it before you ring
If cramps are coming and going, spend an hour writing down when each one starts, how long it lasts, and the gap to the next. It takes an hour and it converts "I think I'm having cramps" into "they are lasting 40 seconds, six minutes apart, for the last hour" — which is the difference between a call that ends in reassurance and a call that ends in being seen. Before 37 weeks, a regular pattern is a reason to ring immediately rather than to finish the hour.

Labour contractions get longer, stronger and closer together and do not stop for any of that. Before 37 weeks, anything that becomes regular is a call regardless of what you think it is.
Can cramping mean miscarriage?
It can, but cramping alone is a weak signal — mild cramping is extremely common in pregnancies that continue normally. What raises the concern is cramping with bleeding, particularly if the bleeding is heavy or contains clots or tissue, and pain that is severe rather than mild. That combination needs assessment the same day, usually at an early pregnancy unit.
What about cramping after sex?
Mild cramping and some spotting after sex are common and usually harmless in an uncomplicated pregnancy — the cervix is more vascular and bleeds easily. It should settle within an hour or two. Cramping that continues, or bleeding that is more than light spotting, still gets a call. And if you have been told you have a low-lying placenta or advised against intercourse, any bleeding afterwards is assessed.
