Breast Tenderness in Pregnancy

Pregnancy at week 6 — when breast tenderness is typically at its peak

For a lot of people this is the symptom that came first — before the nausea, before the test, sometimes before the missed period. Breasts that ache, feel heavy, and object to being touched or knocked.

The short answer: sore, full, tingling breasts with darkening areolas are one of the earliest pregnancy signs, driven by rising oestrogen and progesterone. It usually eases around week 14 as the hormonal curve flattens. Symptoms fading is common and is not, on its own, a sign of a problem. A supportive non-underwired bra is the intervention that actually helps. What needs a call: a hot, red, painful patch on one side, especially with a fever.

Why it happens

Oestrogen and progesterone rise steeply from the moment of implantation. Blood flow to the breast increases, the ductal system starts to develop, and fat is laid down. All of that happens over weeks, in tissue that is not used to changing quickly, and the stretching is what you feel. The areolas darken and widen in the same period, and the small bumps on them — Montgomery glands — become more prominent.

The reason this is a poor home pregnancy test is that progesterone also rises in the luteal phase of every ordinary cycle, which is why sore breasts are a familiar premenstrual symptom. If you are reading this before a positive test, the early pregnancy symptoms quiz covers which signs distinguish the two and when a test becomes reliable.

What is normal

NormalGet it looked at
Distribution Both breasts, general soreness and fullness One breast, one area, hot or red
Feel Heavy, tingling, tender to pressure Hard, throbbing, severe — with fever or feeling unwell
Skin Darker areolas, visible veins, prominent glands Dimpling, puckering, or an orange-peel texture
Lumps Generally lumpier, changing week to week One firm lump that stays put over weeks
Discharge Clear or yellowish colostrum later in pregnancy Blood-stained discharge from one nipple

The symptoms-have-stopped question

This is the reason many people arrive on a page about sore breasts, so it is worth answering plainly. Pregnancy symptoms easing in the first trimester is normal, extremely common, and in the great majority of cases means nothing except that hCG has stopped doubling. It is mentioned in connection with miscarriage because it can occur alongside one — but so can having no change in symptoms at all, and the symptom that actually carries information is bleeding or cramping, not the absence of soreness.

Two useful things: symptoms rarely vanish in a single morning, so a gradual fade over a fortnight is reassuring; and if you are anxious enough to be reading this at 2am, ring your midwife in the morning. Early pregnancy units exist for exactly this, and nobody there considers it a waste of time.

Call the same day if: one breast has a hot, red, hard or intensely painful area, particularly with a fever or feeling generally unwell — that is an infection and it is treated with antibiotics. Also ring, but not urgently, about a firm lump that persists over several weeks, skin dimpling or puckering, or bloody discharge from one nipple. Pregnancy is not a reason to defer examining any of those.

Breast tenderness in pregnancy — at a glance

Rising oestrogen and progesterone increase blood flow and begin building milk-producing tissue, making sore, heavy, tingling breasts with darkening areolas one of the earliest pregnancy signs — often the first, sometimes before a missed period. Because the same hormones rise in every luteal phase, it does not distinguish pregnancy from an approaching period. It typically eases around week 14 as the hormonal curve flattens, while the size and heaviness remain. Symptoms fading in the first trimester is common and is not on its own a warning: bleeding or cramping is what carries information, and a gradual fade over a fortnight is reassuring. A well-fitted non-underwired bra, remeasured as the size changes, is what actually helps, along with a soft sleep bra if the pain wakes you. A hot, red, hard or severely painful area on one side, especially with fever, is an infection and needs a same-day call; a persistent firm lump, skin dimpling or bloody nipple discharge needs examining regardless of the pregnancy.

Frequently Asked Questions

When does breast tenderness start in pregnancy?

Often very early — for many people it is the first sign, arriving in the weeks around a missed period and sometimes just before one. Rising oestrogen and progesterone increase blood flow and start building the milk-producing tissue, and the tissue is stretching faster than it is used to. That is also why it is not a reliable pregnancy test: the same hormones rise in the second half of an ordinary cycle, which is why premenstrual breasts feel similar.

When does breast tenderness stop?

Usually it eases around the end of the first trimester, commonly by about week 14, as the rate of hormonal change slows. The breasts stay larger and heavier — that part continues — but the raw soreness typically settles. Some people find it returns later in pregnancy as the breasts prepare for feeding, and that later version is more fullness than pain.

My breasts have stopped hurting. Does that mean something is wrong?

Almost always no. Symptoms easing in the first trimester is common and is usually just the hormonal curve flattening — most people whose symptoms fade have entirely healthy pregnancies. The reason it frightens people is that a sudden loss of all pregnancy symptoms is sometimes mentioned alongside miscarriage. On its own, without bleeding or cramping, it is weak evidence of anything. If you are worried, ring — that is what the number is for.

What can I do about sore breasts in pregnancy?

A well-fitted non-underwired supportive bra makes the most difference, and getting measured is worth doing more than once, because the size changes. A soft crop top or sleep bra at night helps if the pain wakes you. Warmth or cool packs, whichever suits, and avoiding pressure on them during sleep. Paracetamol is available if it is genuinely stopping you sleeping.

When is breast pain in pregnancy a warning sign?

When it is one-sided and localised rather than general: a hot, red, hard or exquisitely painful patch, especially with a fever or feeling unwell, which suggests infection. Also worth showing someone: a new lump that does not change over a cycle of weeks, skin dimpling or puckering, or any bloody nipple discharge. Pregnancy makes breasts lumpy and that is normal — but "it is just pregnancy" is not a reason to leave a discrete lump unexamined.
